Requirements

The following steps must be completed before you are eligible to be in the Certified Nurse Aide (CNA) program:

• Certified Nurse Aide inquiry form
  Return the required documents listed below in person to the Donnelly College Admissions Office: 608 North 18th Street, Kansas City, Kansas 66102.
• Copy of driver's license OR valid photo identification
• Copy of Social Security Card
• Students must provide proof of required immunizations (TB, COVID-19, and Flu when in season). 
• Negative TB test (within last 6 months) OR Chest X-Ray (within last 5 years)
